| STEP ONE - PRE-ENROLLMENT |

| _____ | Complete Pre-Enrollment Form |

| STEP TWO - APPLY FOR FINANCIAL AID |
| _____ | Apply online at www.fafsa.ed.gov (Lively Technical Center Code - 013997)
Apply early. However, you may apply at any time during the school year. Please contact the Financial Aid Office at 850-487-7592 to verify eligibility two weeks after submission. |
| _____ | The following documents will be required: tax return, driver license, social security card, and proof of Florida residency for 12 months. |

| STEP THREE - VISIT STUDENT SERVICES |
| _____ | All new students are required to meet with an Advisor prior to enrollment. Call to schedule an appointment with Student Services at 850-487-7473 or visit Student Services. |
| _____ | Advisor will determine if testing is necessary. |
| |
| STEP FOUR - ASSESSMENT TESTING |
| _____ | Testing is available in Student Services Monday - Thursday, 7:45 a.m. - 4:00 p.m.
Note: testing must begin no later than 1:00 p.m. |
| _____ | Bring a picture ID and allow two to three hours to complete the test. |
| _____ | Testing Fee is $15.00. Payment is due prior to testing at Registration in Building 8. Cash, MasterCard, Visa, money order, and cashier's check are acceptable forms of payment. |
| _____ | Review test scores. Finalize action plan. |
| _____ | If financial aid is not needed, complete Admissions Packet and go directly to STEP SIX. |

| STEP FIVE - VISIT FINANCIAL AID OFFICE |
| _____ | Bring Admissions Packet all documentation utilized for completing the FAFSA application with you. Documentation must be verified by Financial Aid. |

| _____ | Complete Admissions Packet. |

STEP SIX - REGISTRATION
(CONGRATULATIONS! YOU ARE NOW READY TO REGISTER!) |
| _____ | Bring completed Admissions Packet to Registration in Building 8. |
| _____ | Acceptable forms of payment as follows: Cash, Visa, MasterCard, money order and/or cashiers check. Or, Financial Aid deferral document from sponsoring agency. |
Registration & Student Services Hours of Operation
- Monday - Thursday 7:45 a.m. - 4:00 p.m.
- Friday 7:45 a.m.- 3:00 p.m.
- For the week proceeding each quarter and for the first week of classes, Registration and Student Services will be open from 7:45 a.m. until 6:00 p.m. Monday through Wednesday.
- Summer and holiday hours may vary and will be posted.
